What do I need to do before a surgery?

Before a surgery, please go to the Main Reception of the Hospital da Ordem Terceira do Chiado, take the corresponding number, and follow the established formalities.
You will be asked to:
» Fill out some forms;
» Surrender the Consent Form of one of the Entities with an agreement with the Hospital da Ordem Terceira do Chiado, should such entity have failed to timely send it to the Hospital;
» Pay an advance corresponding to the surgery at issue. That amount shall subsequently be adjusted, when the invoice is issued.
